Effective with the 2010-2011 school year, the Ohio Department of Health recently added additional immunization requirements for school entry for Ohio's Kindergarten and 7th grade students. 

 

The changes in effect for the 2010-2011 school year are:

one booster dose of Tdap (Tetanus, diptheria and acellular pertussis) or Td (Tetanus and diptheria) vaccine will be required prior to the entry to seventh (7th) grade AND the addition of a 2nd dose of varicella (chicken pox) vaccine for kindergarten entry.

The Ashtabula City Health Department is open for childhood / adolescent vaccinations every Monday & Wednesday from 9:00 a.m to 11:00 a.m.

H1N1 vaccinations are open to everyone interested in receiving protection from the H1N1 influenza virus.

Immunization:   Tetanus Booster, Influenza, Pneumonia,  Hepatitis B, Hepatitis A, HPV, MMR, ZOSTAVAX

Tuesday & Thursday 9:00 a.m. - 11:00 a.m. Walk-in, first-come basis.

Medicare & Medicaid payment accepted for most vaccines.  For current vaccine prices contact the nursing division.

Blood Pressure Screening:  Tuesday & Thursday 9:00 a.m. - 11:00 a.m. for the general public at no charge. 

Cholesterol and Glucose Screening:  Tuesday & Thursday 9:00 a.m. - 11:00 a.m.

Available:   Total Cholesterol, Good Cholesterol and Bad Cholesterol, Triglycerides, and Blood Sugar.

Cost: $25.00 for Lipid Panel (Blood Sugar, Glucoscan - $2.00)

Tuberculosis Skin Tests:   Monday & Tuesday 9:00 a.m. - 11:00 a.m.

Cost: $7.00 per test.  Medicaid accepted.

Must return in 48 hours for reading of skin test.

Tuberculosis Clinic:   Held the second and fourth Thursday of the month, call your local Health Department for referral.

Chronic Disease Visits:   Home and office visits with physician's orders.

Health Education:   Services are available throughout the county along with pamphlets and videos.

Services for Children & Adolescent

Immunization:   All required Infant Shots, Diphtheria, Pertussis, Tetanus, Haemophilus B (Hib), Polio, Measles, Mumps, Rubella, Varivax, Rotavirus, Hep A, Meningitis, HPV Gardasil, Tdap (Tetanus Booster) for adolescents, Hepatitis B, Pneumococcal.

Monday & Wednesday 9:00 a.m. - 11:00 a.m. Walk-in, first-come basis

Donation of $5.00 per vaccine.   Medicaid accepted. Donation may be waived if unable to pay. 

Communicable Disease:  Office and home visits for assistance in identification, reporting and education.

Well Child Physical Exam:  Fee on sliding scale or Medicaid payment.  Bimonthly clinics held for children ages birth to 21 years.  Call 440-992-7122 for an appointment.

Bureau for Children with Medical Handicaps:  Diagnostic referrals and appointment with follow-up visits as needed.  Contact Health Department for further information.

Blood lead testing:  Contact Health Department for an appointment.

Lead Level Elevations:  Elevated lead level reporting.  Follow-up assessments and education with the Environmental Health Director.

Orthopedic Clinic:  County-wide clinic by appointment, referral by physician, school nurse or public health nurse.  Contact Health Department for further information.

Vision and Hearing Screening:  Contact Health Department for an appointment.
