Birth & Death Certificates

 Vital Statistics 

The Ashtabula City Health Department has birth and death records on file for persons who were born or who died in Ashtabula City from about 1909 to the present.   You can come into the office Monday through Friday between 8:00 a.m. - 12:30 p.m. and 1:30 p.m. - 4:30 p.m. to obtain a certified copy of the desired certificate.  We can process your request while you wait or you can request the certified copy by mail.   A certified copy of a birth or death certificate has the raised seal and the signature of the Registrar of Vital Statistics.  The cost for a certified copy is $19.00.   Uncertified copies are no longer available.

If you need to make a request by mail simply complete either the Birth or Death form and return it with a $19.00 check or money order along with a stamped, self-addressed envelope.  You may mail your request to:

Please allow 5 to 7 days for the return of your certificate.   

 

We now accept Visa & Mastercard.   You must fax us a copy of your credit card and signature to 440-992-7163.  You will be charged $19 for the certified copy  and postage.   You may choose postage of $.42 for regular mail; $4.80 for priority mail; or $16.50 for overnight mail.  Please be advise that the fees are charged according to what the USPS is charging at the time of order.

Genealogy Search 

We are glad to assist you in your search for your family genealogy.   All we need are the persons' names and approximate dates of birth or death.  We will search our files and allow you to view the documents related to your request.  While all death records are open for review, birth records are limited due to the confidential information they contain.
