Please note: All patients over the age of 2 years will be required to wear a mask and pass a screening questionnaire to be seen by the nurse.  One caregiver per patient is allowed.

Influenza Vaccinations every Monday from 2-4PM Drive-thru clinic located at the Municipal Building, 4717 Main Ave. Ashtabula.  Please bring your insurance for billing. Decrease your wait time by completing this form and bringing it with you!


Influenza vaccinations are also available by appointment in the Health Department; please call 440-992-7122 to make an appointment.

Mantoux testing (tuberculosis skin test)

  • must return 48 hours later for reading of skin test.

Children with Medical Handicaps Program

  • The CMH program is administered by the Ohio Department of Health.  The program serves children with special health care needs and their families.  The goal of the CMH program is to assure that children with special health care needs and their families obtain comprehensive care and services.  Visit the Ohio Department of Health website for more information on the program. & type BCMH in the search box.
